If you have been referred for an ambulatory sleep study and booked an appointment at NCSS for this sleep study you will be required to fill out three questionnaires. These are contained in the downloadable document below. For your convenience you may download and fill out the questionnaires prior to attending the clinic. To return them you may upload them via our website on the link below once complete, or bring them with you to your first appointment.
